For the treatment:

You’ll lie down on a hospital bed. An imaging method, such as ultrasound, is used to guide the procedure.
The leg to be treated is injected with numbing medicine. Once your leg is numb, a needle makes a small hole (puncture) in the vein to be treated.

The catheter containing the laser heat source is inserted into your vein.
More numbing medicine may be injected around the vein. Once the catheter is in the right position, it's then slowly drawn backward.
As the catheter sends out heat, the vein is closed off.
In some cases, other side branch varicose veins may be removed or tied off through several small cuts (incisions).
When the treatment is done, the catheter is removed.
Pressure is applied to the insertion site to stop any bleeding.
An elastic compression stocking or a bandage may then be put on your leg.
The treatment takes 45 to 60 minutes, you can go home the same day.

What is EVLT?

Venous laser treatment (EVLT) is a procedure that uses laser heat to treat varicose veins. It is a minimally invasive
procedure that makes use of catheters, lasers, and ultrasound to treat varicose veins. This procedure is performed most often on veins that are still relatively straight and untwisted.

Once closed, the blood flow is naturally redirected to healthier veins.

.The 1470nm laser with significantly higher absorption in water, provides a superior precision instrument for reduced collateral thermal damage around venous structures.Accordingly, it is highly recommended for endovascular work.

Laser wavelength 1470 is, at least, 40 times better absorbed by water and oxyhemoglobin than 980nm laser, allowing selective destruction of the vein, with less energy and reducing side effects.

As a water-specific laser, the TR1470nm laser targets water as the chromophore to absorb the laser energy. Since the vein
structure is mostly water, it is theorized that the 1470 nm laser wavelength efficiently heats endothelial cells with a low risk of collateral damage, resulting in an optimal vein ablation.

Venous laser ablation (EVLA) treats major varicose veins previously treated by stripping surgery. With ultrasound guidance, a laser fibre is placed into the abnormal vein through a tiny incision. The vein is then numbed with local anaesthetic, and the laser activated as the fibre is slowly removed. This produces a reaction in the vein wall along the treated section, resulting in collapse and sclerosis of the vein wall with minimal discomfort.

The published success of EVLA treatment is between 95-98%, with far fewer complications than surgery. With the addition of EVLA to ultrasound guided sclerotherapy, it is expected that varicose vein surgery will be performed much less frequently in the future.

Treatment Principle

Venous Laser Ablation Treatment, also known as laser therapy, is a safe, proven procedure that not only treats the symptoms of varicose veins.

Venous means inside the vein, a small amount of local anaesthetic is injected into the skin over the vein and a needle inserted into it. A wire is passed through the needle and up the vein. The needle is removed and a catheter is passed over the wire, up the vein and the wire removed. A laser fiber is passed up the catheter so its tip lies at the highest point to be heated (usually your groin crease).

A large quantity of local anaesthetic solution is then injected around the vein through multiple tiny needle pricks. The laser is then fired up and pulled down the vein over to heat the lining within the vein, damaging it and causing it to collapse, shrink, and eventually disappear.
