    What is EVLT?
    EVLT was pioneered in 2003. It was the first viable alternative to surgery as a treatment for venous ablation. Surgery typically consisted of a combination of stripping, or vein removal, and ligation, literally tying the vein closed. Almost all vein surgery involved multiple excisions and required a prolonged recovery period. EVLT only required one very small incision, and patients are able to walk out of the office on the same day. The rates of thrombosis (blood clots) and infection was significantly lower than with surgery. Now long term data shows that recurrent vein disease is much less likely with endovenous ablation than with any surgical intervention.
    Endovenous laser treatment (EVLT) is a procedure that uses laser heat to treat varicose veins. It is a minimally invasive procedure that makes use of catheters, lasers, and ultrasound to treat varicose veins. This procedure is performed most often on veins that are still relatively straight and untwisted.
    Through a small puncture hole on the skin, a fibre is thread into the varicose vein under ultrasound guidance. Laser energy is released precisely from the fibre to ablate and seal off the diseased vein.

    Laser interaction with tissue depends on the wavelength of choice. The Laseev laser comes in 1470nm.
    The 1470nm laser with significantly higher absorption in water, provides a superior precision instrument for reduced collateral thermal damage around venous structures.Accordingly, it is highly recommended for endovascular work.
    Laser wavelength 1470 is, at least, 40 times better absorbed by water and oxyhemoglobin than 980nm laser, allowing selective destruction of the vein, with less energy and reducing side effects.
    As a water-specific laser, the TR1470nm laser targets water as the chromophore to absorb the laser energy. Since the vein
    structure is mostly water, it is theorized that the 1470 nm laser wavelength efficiently heats endothelial cells with a low risk of collateral damage, resulting in an optimal vein ablation.

    Which method is the best for treatment of reticular and spider veins, sclerotherapy or laser?
    The optimal method at the present time is sclerotherapy. For the pink and purple spider veins with liquid sclerosant, for the
    green reticular veins with foam. Laser can also be used, but it is effective only when “matting” is present. It’s a situation when after performing sclerotherapy some new tiny pink vessels appear like a pink “cloud”. At larger vessels the laser is not as effective as sclerotherapy.

    How long do the patients need to wear elastic stockings after vein treatment?
    If a varicose veins treatment with laser or radiofrequency ablation accompanied with miniphlebectomies then the period of elastic compression should be 3 weeks. At some cases when bioglue is used it can be shorten to few days or even no compression at all. After sclerotherapy the elastic compression period is 5 days.

    May you treat both legs at the same time?
    The only restriction that we have is the daily dosage of the drug that we inject. The overdosing can cause serious side effects. This is the reason the treatment is being divide to sessions that can be repeated every 48 hours, in practice twice a week. The total number of sessions needed is strictly individual and depends on the severity of the problem.

    Are the results permanent? If not, how long does it last?
    The veins that are being sclerosed don’t reappear. However, since venous insufficiency is a chronic disease there is a strong possibility that new veins can reappear at new spots or even at the ones that have been treated previously. The recurrence rate can be minimized with prophylactic measures, such as sport activity, avoiding of long standing, body weight management, use of elastic compression during long period of standing or sitting etc.

    Which patients should perform a prophylactic scan of carotid arteries or check up for aneurysm disease?
    Patients over 50 years old with smoking history or high cholesterol or family history of atherosclerosis or aneurysm.

    Can sclerotherapy take place during pregnancy or breast feeding?
    No, pregnancy and lactation are contra indications for sclerotherapy.


    Application


    During the EVLA procedure, the surgeon uses ultrasound to find the vein to be treated. The veins that can be treated are the main venous trunks of the legs:
    Great Saphenous Vein (GSV)
    Small Saphenous Vein (SSV)
    Their major tributaries such as the Anterior Accessory Saphenous Veins (AASV)

    Endovenous laser ablation (EVLA) treats major varicose veins previously treated by stripping surgery. With ultrasound guidance, a laser fibre is placed into the abnormal vein through a tiny incision. The vein is then numbed with local anaesthetic, and the laser activated as the fibre is slowly removed. This produces a reaction in the vein wall along the treated section, resulting in collapse and sclerosis of the vein wall with minimal discomfort.

    The published success of EVLA treatment is between 95-98%, with far fewer complications than surgery. With the addition of EVLA to ultrasound guided sclerotherapy, it is expected that varicose vein surgery will be performed much less frequently in the future.




    Treatment Principle


    Endovenous Laser Ablation Treatment, also known as laser therapy, is a safe, proven procedure that not only treats the symptoms of varicose veins.
    Endovenous means inside the vein, a small amount of local anaesthetic is injected into the skin over the vein and a needle inserted into it. A wire is passed through the needle and up the vein. The needle is removed and a catheter is passed over the wire, up the vein and the wire removed. A laser fiber is passed up the catheter so its tip lies at the highest point to be heated (usually your groin crease). A large quantity of local anaesthetic solution is then injected around the vein through multiple tiny needle pricks. The laser is then fired up and pulled down the vein over to heat the lining within the vein, damaging it and causing it to collapse, shrink, and eventually disappear.
